說明：
一、依據本局 113 年 1 月 10 日桃教特字第 11300004172 號函及 113 年 1 月 25 日桃教特字第 11300038232 號函辦理(計達)。

二、因應該中心編制及業務異動，該中心提供旨案服務至 113 年 7 月 31 日止結束。

三、本案服務資訊請貴校(園)公告周知，後續教育輔具等相關服務(如各校身心障礙學生教育輔具評估、專業人員駐點諮詢、輔具調整、實地到校或到宅輔導及維修申請等服務)將移轉至東門國小(本市國小特殊教育資源中心)辦理預約教育輔具駐點服務與諮詢事宜，如有疑問請洽東門國小(本市國小特殊教育資源中心)謝老師，聯絡電話：(03)3394572 分機 836 。
